Submitting Claims

Dental claims

Great-West is electronically linked with many dentists across Canada. If your dentist has such a link with us, your claim may be settled right in the dentist's office. The dentist submits the claim electronically and receives confirmation of coverage. You only pay for any expenses not covered by Sonata Health.

To submit a paper claim form:

1. Complete the Dentalcare Claim Form. [PDF]
2. Mail the completed form and original receipts to the Individual Health Unit.

Healthcare claims

For all healthcare expense claims:

1. Complete the Healthcare Expenses Statement. [PDF]
2. Mail the completed form and original receipts to the Individual Health Unit.

Hospital Accommodation claims

If you purchased the Hospital Accommodation Benefit or you have the Guaranteed Acceptance Plan (GAP), Great-West pays benefits for semi-private accommodation in hospital and accommodation in nursing homes.

Most hospitals and nursing homes use their own claim forms and will submit your claim directly to Great-West on your behalf. You’ll need to supply the hospital or nursing home with your Sonata Health policy number.

If the hospital or nursing home won’t submit the claim on your behalf:

1. Complete the Healthcare Expenses Statement. [PDF]
2. Mail the completed form, along with your original receipts, to the Individual Health Unit.

Hospital Cash claims

If you purchased the Hospital Cash Benefit, you’ll receive your cash benefit automatically when you submit your claim for other hospital expenses. You don’t have to file a separate claim or provide receipts.

Accidental Death, Dismemberment and Specific Loss claims

To make a claim for Accidental Death, Dismemberment and Specific Loss benefits, contact your financial security advisor or the Individual Health Unit. They will help you make a claim.
