Prior Authorization Forms
Certain prescription drug claims need to be approved before we can consider reimbursement. Our Prior Authorization process helps us ensure that appropriate drug coverage is provided, and that prescribed drugs are considered reasonable treatment for your condition.
If you want to be considered for coverage for one of the drugs listed, and your group benefits plan covers that drug, have your attending physician complete the Request for Information form for the appropriate drug, and then send the form to Great-West.
